These tools are used to assist service delivery and do not replace professional judgement. All documentation and recommendations are reviewed and approved by a qualified clinician prior to finalisation. We take reasonable steps to ensure that any tools used manage Personal Information in accordance with the Privacy Act and the Australian Privacy Principles.

Where reasonable and practicable to do so, we will collect your Personal Information only from participants. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

We do not charge a fee for access requests, however an administrative fee may apply for the provision of copies. Identification may be required prior to release of information.
